Hearts are the standard health for most characters, and can be found in several varieties as pickups.

Fiend Folio adds 10 new kinds of hearts, which either add pre-existing types of health to the health bar, or add entirely new health types with unique functions.

  • Grants a full Immoral Heart which is added at the end of the health bar after RedUI.png Red Heart containers, similarly to Soul Heart.png Soul Hearts.
    • Immoral Hearts will replace Soul Hearts in the health bar when at the maximum amount of hearts, and can likewise be replaced by Black Heart.png Black Hearts, but can not be replaced by Soul Hearts.
    • Full Immoral Hearts will heal half of a Black Heart or half of a Soul Heart in the health bar, leaving half of an Immoral Heart.
  • When the player takes damage to their Immoral Hearts while in an uncleared room, all adjacent Immoral Hearts remaining in the health bar are removed and turned into FiendishFriends.png Immoral Minions.
    • Upon clearing the room, all Immoral Minions reform back into Immoral Hearts in their pickup form.
    • Immoral Hearts that are not next to each other in the health bar will not be converted to Immoral Minions.

Morbid Heart

  • Fills RedUI.png Red Heart containers or Bone Heart Containers with a Morbid Heart, replacing Red Heart.png Red Hearts and Rotten Heart.png Rotten Hearts if at full health.
  • Each Morbid Heart in the health bar takes three hits to be fully depleted, being equivalent to 1 and 1/2 hearts of health.
    • Against damage sources that deal a full heart, such as in Chapter 4 and onwards, taking a hit will remove two-thirds of a Morbid Heart.
    • Partially depleted Morbid Hearts in the health bar can be restored with Red Hearts, Rotten Hearts, or other Morbid Hearts. Each half a heart of healing restores 1/3 of a Morbid Heart.
    • When a Morbid Heart is fully depleted, it spawns a "Morbid Chunk", a temporary orbital familiar that blocks shots and deals 7 contact damage per tick. A Morbid Chunk is typically destroyed after blocking 3 projectiles, and a player can only have up to 3 Morbid Chunks at once.

Notes

  • Immoral Hearts and Morbid Hearts have a chance to appear naturally from random heart drops:
    • Immoral Hearts have a 0.5% chance to appear, roughly equivalent to that of Black Hearts. However, having an Immoral Heart in the health bar increases the chance for Immoral Hearts to drop to 2% — not including FiendCoopIcon.png Fiend, as he has his own separate chance for Immoral Hearts to appear.
    • Morbid Hearts have a 0.8% chance to appear, about less than half that of Rotten Heart.png Rotten Hearts.
      • Only full variants of Morbid Heart pickups will appear naturally — 2/3rd Morbid Hearts and 1/3rd Morbid Hearts will not spawn, and only appear in certain rare rooms such as Icon Super Secret Room.png Super Secret Rooms.
      • 1/3rd Morbid Hearts will replace any spawned 1/2 Red Hearts when picking up Cursed Penny FF.png Cursed Pennies while holding Bloody Penny.png Bloody Penny.
  • Blended Black Hearts and Half Black Hearts do not appear from random drops naturally, and will almost exclusively appear while playing as FiendCoopIcon.png Fiend or BastardCoopIcon.png The Bastard, or through using the ThreeOfHearts.png 3 of Hearts card.
    • Blended Immoral Hearts, meanwhile, only appear for FiendCoopIcon.png Fiend, or through using the ThreeOfHearts.png 3 of Hearts card.
  • Unlike Rotten Hearts, Morbid Heart damage caused by enemies or most hazards will count as Red Heart damage, and will act as penalty against the chance for a Devil Pool Icon.png Devil Room or Angel Pool Icon.png Angel Room to appear.

With new heart pickups come new special Clots for the Sumptorium.png Sumptorium item.

Trivia

  • The name for Immoral Hearts are based off of the Immortal Hearts from Antibirth, a mod for The Binding of Isaac: Rebirth and the predecessor to the Repentance DLC.
    • The name was conceptualized before Repentance released, during which time the Fiend Folio developers assumed that Immortal Hearts would be added to the DLC — however, this never happened.
  • Picking up an Immoral Heart has a 0.1% chance to play a different higher-pitched sound.
